Greetings! This page contains links and forms for those being considered for Ordination or Recognition at 2026 District Advance.

CANDIDATES FOR ORDINATION:

Application forms:

Forms are available in fillable WORD or PDF (not fillable) formats. PLEASE do not send “pages” files (convert to PDF). You are required to complete an Application for District License (in the event you are not recommended for ordination this year) AND an Ordination application. Please download the forms and rename them before completing them, then save again.

SUBMITTING FORMS:

You will use the online scheduler (see link below) to set up your appointment time. The system will required you to attach the complete applications. The submission deadline is October 9, 2025. You will receive a confirmation email. We have a limited number of evening appointments, so if at all possible, please arrange for a daytime slot. Slots listed as En Español will have Spanish speakers on the interview committee.

You will need to complete ONLY the Application for Ordination/Recognition form above. The scheduler will ask for two forms, feel free to attach the Application twice.
